We only collect health information to underwrite life and long-term care insurance; process claims; annuities; or other insurance or financial products if needed, and we obtain your written permission before it is collected. If you contact another company to apply for life or long term care insurance or make a claim, the MIB may supply that company with the information it has in its files.

Using and Sharing Your Information

To further protect you we limit our use and sharing of your information to provide and maintain products for you, or as permitted or required by law. We may share your information to:

     1. Process and service your business: underwrite, issue or administer and service your contracts, benefits or
         accounts provided by Treestreams.com or other companies through which we have a joint marketing or

          with a legal interest in your contract, benefit or account) or, directed by a law enforcement,

If you are covered under a group contract, we only share your information in order to administer your coverage.

     • Non-affiliated people providing benefits and services on your behalf (e.g., agents, brokers, financial advisors,
        plan administrators / sponsor for group benefits, etc.) and,
     • Companies providing services (e.g., underwriting, reinsurance, claims processing, marketing fulfillment services,
        computer processing, data hosting, printing, design, etc.) directly to us or on our behalf.

We require that non-affiliated persons or companies use your personal information only to perform their function, and to keep such information confidential. They are not permitted to use your personal information for their own marketing purposes.

After your business relationship with us has ended, we may share your information with your agent or broker, or as permitted or required by law.

Reviewing and Updating Your Information

Generally, you have the right to review the information we have collected about you. We may limit this right if a claim or legal proceeding is pending.
